Tests must be scheduled at least 2 business days in advance. This form will only allow advance scheduling.

Testing may only be scheduled Monday - Friday 8:30am - 3:00pm. Please enter a start time on the hour or half-hour only (e.g. 11am or 11:30am, not 11:45am). The office closes at 4:30pm and all tests must be completed by that time. We will do our best to accommodate your requested exam start time. If we are booked to capacity during your requested time, we'll contact you for an alternative time.
